Insurance Information

Bring Your Insurance Cards to Every Appointment

Please present your current insurance card or cards at Central Check-in when you arrive for your appointment. Our staff will verify that we have correct information for you.


If you have an insurance co-payment, it will be collected at Central Check-in.

Automatic Filing

You can arrange with the staff at Central Check-in for automatic filing of primary and secondary insurance.
