Sunforest Court is located off Sylvania Avenue between Notre Dame High School and the Medical Mutual of Ohio Building. Our building is the second on the left. Take the elevator or stairs to the second floor.
Hours of Operation
Monday - Friday, 8 a.m. to 5 p.m.
Evening and early morning appointments may be available upon request.

We are located between Airport Highway and Angola, right across the street from the Holland Library.
From I-475, exit on Airport Highway-West. Turn right onto McCord Road (~one mile from I-475). Continue on McCord Road past Springfield High School on the left, and over the railroad tracks. The CHIP clinic is the fifth drive way on the right side of McCord past the railroad tracks. The complex is called McCord Plaza.

The CHIP Program runs from 8:30 – 2:30 Monday through Thursday. Please leave a detailed message and your call will be returned within one business day.
