About Your Visit
We look forward to serving you at the Mercy Eye Center. When you come to your appointment please remember to bring your:
- glasses and/or contact lenses
- health insurance card
- Medicare/Medicaid card (if applicable)
- social security card
- physician referral (if appropriate)
- driver’s license
- a list of medications you are currently taking (with dosage amounts)
If you are a new patient please
print out and complete the New Patient Form prior to your visit and bring it with you the day of your appointment. If you are coming for a pediatric appointment or if you are seeking treatment for adult strabismus
please print and complete this form as well.
Plan on your appointment lasting for between 1 – 2 hours depending on the type of procedure or services you require. Because your eyes are likely to be dilated as part of your comprehensive eye exam or outpatient procedure you will need to be accompanied by someone who can drive you home or make arrangements for other transportation.
Insurance and Payment Policy
We participate in most major insurance plans and accept Medicare and Medicaid claims. All insurance co-pays are required at the time of service. We will file the claim with your insurance however you will be billed directly by your insurance company for any services that are not covered under your plan. If your insurance requires that you get a referral from your general physician you – or your doctor’s office - will need to send that completed referral to our office a day before your appointment. If we do not have a referral on file at the time of your appointment we will need to re-schedule. If you do not have health insurance, please inform us prior to your scheduled appointment.
